Abstract

Metabolic syndrome is a multiplex risk factor that arises from insulin resistance accompanying abnormal adipose deposition and function (Olufadiet al.,2008). It is a risk factor for coronary heart disease, as well as diabetes, fatty liver, and several cancers.
metabolic syndrome is associated with a chronic, low-grade inflammation (Grundy et al., 2005). Several pro-inflammatory cytokines have been shown to be elevated in parallel with an increasing number of components of the syndrome, whereas the anti-inflammatory and adipocyte-specific substance adiponectin is consistently lower (Kowalska et al., 2008; Hung et al., 2008).
Increasing levels of circulating IL-18 have been reported to be closely associated with the components of MetS and to predict type 2 diabetes, cardiovascularevents, and mortality (Blankenberg et al., 2003; Everett et al., 2009) .
So this study aimed to assess serum level of IL-18 metabolic patients compared to premetabolic patients and healthy subjects.
the result of this study showed that levels IL-18 were significantly higher in metabolic group compared to its levels in normal group , Levels of IL-18 were significantly higher in premetabolic group compared to its levels in normal group and Levels of IL-18 were significantly higher in metabolic group and compared to its levels in premetabolic group .
concentrations of IL-18 in this study was significantly positively correlated with waist circumference, systolic blood pressure , diastolic blood pressure, fasting blood sugar and HDL level in metabolic and premetabolic groups.
In conclusion IL-18 has been elevated in subjects with the metabolic syndrome and to increase in parallel with an increasing number of components of the syndrome. using ROC curve IL-18 serum level can be used as screening for metabolic syndrome with a cutoff value greater than 210 pg/ml ,while a cutoff value greater than 250 pg/ml can be used for diagnosis of patient with metabolic syndrome.
