Abstract

Brain dysfunction is common in patients with advanced liver disease.
This study was done to detect any changes in intracranial hemodynamics in patients with liver cirrhosis with or without hepatic encephalopathy The study comprised 30 patients with liver cirrhosis, 10 patients with compensated liver cirrhosis, 10 patients with decompensated liver cirrhosis without hepatic encephalopathy and 10 patients with decompensated liver cirrhosis with hepatic encephalopathy. Ten healthy persons served as control.
For all participants, thorough clinical and laboratory investigations upper endoscopy (for patients only), abdominal sonography and color-coded transcranial duplex Doppler sonography (TCD) were done. The arteries examined were the middle cerebral arteries. The parameters studied in each artery were: peak systolic volume (PSV), end diastolic volume (EDV), and resistive index (RI).
In this study, each group of patients was compared to each other and compared to control group. The results indicated that, cerebral vascular resistance indices measured by transcranial Doppler were increased in association with the severity of cirrhosis and encephalopathy. No change was noticed when the paired arteries were compared indicating absent laterilization.
The cerebral dysfunction was increased with increased the severity of the liver disease, with age, anemia, and abnormal liver function tests
