Abstract

Synovitis is considered to be the earliest event in RA. Also, involvement of the articular cartilage in RA is always secondary.,to synovial inflammation. The articular catilage affection in RA mostly leads to fun­ctional alteration of joints. However, rheumatoid syn­ovitis still has profound consequences even before cartilage destruction is clinically detectable. These functional consequences include decreased resiliance of cartilage, decreased cartilage lubrication, increased synovial lining blood flow, decreased flexibility of the synovium and altered properties of synovial fluid as regards decreased pH, lubrication, increased enzymes and pressure • These early changes make the prompt dia­gnosis and treatment of RA imperative • The concept of the synovial membrane excision as a mean of eradicating or controlling disease symptoms in arthritic maladies is far from a new concept. Swett first reported synovectomy as a treatment of rheumatoid disease in (1923) • Since that time the literature cont­ained a flood of papers about the efficiency of the pro­cedure in rheumatoid disease • The principle aim of the operation is removal of all accessible diseased synovium
