Abstract

Our study is a cross sectional study included 70 patients with psoriasis without clinical evidence of arthropathy.
The group of patients subjected to:
Detailed history taking and clinical examination.
Measuring severity and extent of psoriasis using Psoriasis Area and Severity Index (PASI score), Complete blood picture, C reactive protein (CRP), and Erythrocyte sedimentation rate (ESR), and Musculoskeletal ultrasonography mode/grey scale (GS) assessment on the peripheral joints (hands and feet) as a structural indicator) and power doppler (PD) as a sensitive measure of vascular flow,
All data were collected, tabulated and statistically analyzed revealing that 52 patients with psoriasis who were not complaining of any rheumatological manifestations were found to have abnormal findings by US examination, (62.9%) of the studied patients had wrist and hand joint affection, while (18.6%) of them had tendons of wrists and hands affection, and (17.2%) of patients had both joints and tendons affection of wrists and hands.
(21.4%) of studied patients had ankle and feet joint affection, (35.7%) had Achilles tendons and plantar aponeurosis affection, while (22.9%) of patients had both ankle and feet joints as well as Achilles tendons and plantar aponeurosis affection.
Summary
Then the patients were subdivided according to ultrasound findings into two main groups:
group I: (52) patients with ultrasound findings in joints and/or tendons of wrists, hands, ankles, and feet.
group II: (18) patients without ultrasound findings in joints and/or tendons of wrists, hands, ankles, and feet.
We can conclude that enthesitis and synovial hypertrophy are of the early manifestations of psoriatic arthritis.
The presence of, smoking, higher PASI scores, high ESR and CRP, BMI >24 titers in psoriatic patients could be considered as risk factors for the development of psoriatic arthropathy.
