Those patients characterized by reducing spinal mobility and painful functional activity. Purpose: To compare the effectiveness of polarized light versus microcurrent in the treatment of mechanical low back pain. Subjects: Forty five patients with mechanical low back pain for more than 3 months, their age ranged from 20{u2013}40 years. Methods: Patients were distributed into three equal groups 15 per each. Patients in group (A) received polarized light in addition to traditional physical therapy program and medical care for one month. Patients in group (B) received microcurrent in addition to traditional physical therapy program and medical care for one month. Patients in group (C) received traditional physical therapy program and medical care for one month. Pain and functional improvement are measured using serum cortisol level test and disability questionnaire index respectively before and after the treatment period
