Abstract

Background: Critically ill patients can develop a series of complications due to intensive care unit environment; it was grouped as intensive care syndrome (ICS) which affects the three domains of physical ability, cognitive function, and mental health domains that the adult ICU survivors can experience. The aim of the current study was to examine the impact of nursing intervention protocol on the incidence of Intensive Care Syndrome among critically ill patients. Hypotheses: to fulfill the aim of this study, five research hypotheses were formulated. Setting: This study was conducted at the emergency intensive care units affiliated to Cairo University Hospitals. Subjects & Methods: a group of 31 adult critically ill patients are recruited as a study subject within a quasi- experimental (one group pretest-posttest) research design. A nursing intervention protocol was utilized covering the following: early mobility, range of motion exercises, knowledge and practices regarding intensive care syndrome, delirium assessment and prevention strategies. Patients were assessed pre intervention, cared through the implementation of the intervention protocol and then, daily follow up was done from day four till day seven before discharge. Tools for data collection: Assessment sheets of intensive care syndrome among critically ill patients. Results: three hypotheses can be supported. As there was a significant statistical difference before and after implementation of the intervention protocol regarding knowledge (p: .00), practice (p: .00) and delirium scores (p =.001), However, the other two hypotheses can’t be supported. No significant statistical differences were found between depression, anxiety mean scores and incidence of ICS after implementation. Conclusion: implementing the nursing intervention protocol can improve critically ill patient’s outcomes both functionally and cognitively after having critical illness. Recommendations: Ongoing monitoring of critically ill patients for physical, cognitive, and psychological problems in addition to early recognition of delirium, applying preventive measures to all critically ill patients, implementing of early mobility protocols and range of motion exercises are to be considered in the care of such group of patients. As well as replication of this study on a larger probability sample is highly recommended.
