Abstract

The aim of this study was to evaluate the efficiency of the magnetic habit reminder in cessation of thumb sucking habit and to evaluate the change of magnitude of the open bite.
Twenty five subjects with thumb sucking habit and anterior open bite (AOB) were recruited from the outpatient clinic of the orthodontic department, faculty of dentistry, Ain Shams university. The patients were with an age range of 5-12 years.
A comprehensive diagnosis was performed for each case. Digital Lateral cephalometric and panoramic radiographs were taken and traced by the same clinician using ”WebCeph” (version 1.5.0), extra-oral and intra-oral photographs were taken and study models were also taken and scanned by 3 shape scanner. The patient’s records were taken along with impressions for the appliance’s fabrication. When received from the laboratory, the appliances were disinfected, fitted then cemented and adjusted in the patient’s mouth. The patients and their legal guardians were enlightened about the advantages of the appliances, which entailed cessation of thumb sucking habit thus allowing tooth eruption and open bite closure, and giving them the instrucions sheet at time of appliance insertion. A questionnaire to test patient compliance was given to the parents and the child after the first month and at the end of our study. Treatment was consummated once the subjects ends 12 months of follow up regardless the fact that it could be achieved.
Treatment effects were evaluated digitally by comparing pre and post treatment digital cephalogram using WebCeph and analyzing pre and post treatment digital models using 3 shape Ortho Analyzer. The study results showed the patient were compliant with the magnetic habit reminder. The digital cast measurements showed a statistically significant increase in overbite by 3.41 mm, a reduction of overjet by 1.37 mm, and an improvement of the maxillary intercanine and intermolar width by 1.02 mm and 2.15 mm respectively. This might be the result of stop of thumb pressure on the palatal vault. The digital cephalometric measurements showed statically significant reduction of lower anterior facial hight. The maxillary incisors had been uprightened, however, the mandibular incisors had no statistically significant change after treatment. The interlabial gap had statistically significant reduction after treatment while the nasolabial angle recorded no significant change.
1. Treatment with the magnetic habit reminder was effective to stop thumb sucking habit by its dual action- a combination of mechanical and physical barrier.
2. The magnetic habit reminder had excellent patient acceptance.
3. The anterior open bite and increased overjet associated with the habit were significantly corrected using the appliance.
4. Correction of the proclined upper incisors after cessation of thumb sucking.
5. Improvement of soft tissue profile by decreasing the interlabial gap.
1- More clinical trial should be conducted on patients with combined oral habits such as thumb sucking habit and tongue thrusting habit.
2- More trials should be conducted on patients that had failure with other habit breaking appliances.
