Primary care facilities in Egypt are public sector facilities and the issue of IC in public sector facilities is important, because public sector facilities are the only option available to most low-income groups, who constitute the majority of Egypt{u2019}s population. Objectives: to compare items of assessment of IC measures between national and international assessment tool of IC in PHC, assessment of IC measures in PHC by national assessment tool, assessment of knowledge and practice of HCPs in PHC by international assessment tool, explore hand hygiene compliance among HCPs and explore the fulfillment of hand hygiene requirements. Methodology: Health system operation research, observational study. The study was conducted at 3 PHC facilities (urban, semi urban and rural) using national assessment tool, interview questionnaire derived from the international assessment tool which was conducted for all HCPs at the three facilities and hand hygiene observation checklist. Results: The total IC percent scores using the national IC assessment tool at El Zahraa, Masr El Quadema and Shabramant health unit were excellent, very good and fair respectively. There is a miscommunication between IC team and the HCPs regarding employee health program and monitoring of sharp injuries. Almost all the doctors and nurses didn{u2019}t have pre-employment TB skin test. HH compliance was very low among HCPs during the observation time (For doctors, only one hand hygiene practice from 62 hand hygiene chances and 2 hand hygiene (10%) practices from 20 chances for nurses) and all observed hand hygiene practices didn{u2019}t met the criteria of correct hand hygiene practice
