Abstract

Posterior urethral valves (PUVs) are the most common cause of bladder outlet obstruction in infancy that has a great infliction on the future of the renal and bladder function during childhood and thereafter., This study was planned to evaluate and record the various clinical presentations, management, complications, surgical management and long-term outcome of Posterior urethral valves and to demonstrate the spectrum of the disease, by reviewing those patients involved in our study. The data of 30 patients who have been treated for PUV in Cairo university pediatrics hospital (Abo el reisch) between the period of January 2005 and December 2013 has been reviewed retrospectively. Detailed history has been taken, clinical, laboratory and radiological examination were performed for each patient including; serum creatinine, urine analysis and culture, voiding cystourethrogram, DTPA renal scan, and urodynamic studies, we have recorded the last follow-up data upon which we evaluated the current upper and lower tract function and the involved risk factors., All data were analyzed using computer program SPSS release 15. 30 patients were followed up with a mean age at diagnosis of 6.3 months and a follow-up period ranged between 1.8 years to 10 years, with a mean age at the last follow-up of 5.67 years, 16 patients underwent primary valve ablation only using either cold knife, laser or bug bee electrode, 12 patients underwent 1ry vesicostomy and delayed fulguration and 2 patients underwent primary valve ablation but required diversion for persistence of UTI and ureamia., Of the 30 patients 19 patients were positive for lower urinary tract dysfunction(63.3%) and 26 boys were studied for upper urinary tract function, 9 patients (34.6%) had chronic renal insufficiency and 1 patient (3.8%) had end-stage renal disease ESRD at the last follow up
