Abstract

Background: Radical radiotherapy is one of the main modalities in treating primary brain tumors (gliomas). Radiation has side effects including decrease in cognitive functions due to hippocampal irradiation. Aim of the work: To analyze the possibility of sparing the hippocampus from large radiation doses while using 3D conformal radiotherapy (3DCRT) to treat brain gliomas. Patients and methods: This is a dosimetric study of 31 patients who presented to the Radiation Oncology Department, National Cancer Institute, Cairo University from December 2017 to January 2019.Twenty nine patients had cerebral brain gliomas (23 had high grade {u2013}III and IV, and 6 had low grade{u2013} I and II) and 2 patients had brainstem gliomas. Conventional 3DCRT plans were created.The Hippocampus was then contoured following the RTOG 0933 guidelines. A second hippocampal sparing plan was done delivering comparable doses to the planning target volume and organs at risk.The two plans were then compared concerning the percent reduction in contralateral hippocampal dosely in LGGs with good prognosis and expected long survival
