Abstract

Tracheoesophageal fistula (TEF) is one of the most common neonatal emergencies. It is an abnormal connection between the trachea and the esophagus and it is the most common digestive tract malformation due to the faulty separation of embryonic foregut into trachea and esophagus at 5th to 6th embryonic weeks (Kassif et al., 2021).
Aim of study
This study aims to assess the nurses’ performance regarding care of neonates with TEF.
Research questions:
1. Do nurses have a satisfactory level of knowledge regarding care of neonates suffering from TEF?
2. Do nursing staff provide competent nursing care for neonates with TEF?
3. Are there relations between the nurses’ characteristics and their performance regarding care of neonates with TEF?
1) Research technical design
1- Research design:
A descriptive design was utilized for conducting this study.
2- Research Settings:
This study conducted at neonatal intensive care units (NICU) in children hospital affiliated to Ain Shams University and Sohag University Hospital.
Study subjects:
A convenient sample composed of 70 nurses was included in the study from the previously mentioned settings. 35 nurses from NICU affiliated to Ain Shams University Hospitals and 35 nurses from NICU affiliated to Sohag University Hospitals. The study included nurses from both genders, with different age groups, educational levels and years of experience.

A. Field work:
The actual field work and data collection were achieved through a period of six months started from beginning of January 2021 to the end of June 2021. At the beginning, the researcher introduced herself to the studied nurses and explained the purpose of the study to gain their cooperation and to assure the studied nurse about the anonymity of their answers and that the information will be used for scientific research only and was being strictly confidential.
The data collected through questionnaire and observation check lists to identify nurses’ background information and evaluate their practice. The collection of data was conducted three days per week at morning and afternoon shifts in the previously mentioned settings during providing direct care to neonates by rotation. Each questionnaire took a time ranged from (20: 30) minutes and filling each observation checklists took a time ranged from (45: 60) minutes.
The studied nurses were observed and evaluated for the actual care provided to the neonates having tracheoesophageal fistula by the observation checklists which filled by the researcher.
Results:
The findings of the current study can be summarized as the following:
• More than half (51.4%) of the studied nurses were in the age group of 25 < 30 years ( ±SD 29.64±6.44 years).
• Nearly, half (48.6%) of the studied nurses were technical nursing institute graduates.
• More than half (54.3%) of the studied nurses had not previous training courses about care of neonates having tracheoesophageal fistula.
• More than half (55%) of the studied neonates their chronological age ranged from 1 to < 10 days ( ±SD 10. 50±2. 09 days).
• More than two fifths (45%) of the studied neonates their birth weight ranged from (2500 < 3000 gm).
• According to medical diagnosis about two fifths (40%) of the studied neonates diagnosed with EA.
• According to total knowledge. Only fifth (20%) of the studied nurses had a good level of total knowledge regarding care of neonates having tracheoesophageal fistula. More than third (37.1%) of them had average level of total knowledge. While, more than two fifths (42.9%) of them had a poor level of total knowledge.
• More than two fifths (45.7%) of the studied nurses had competent level of total practices regarding care of neonates having tracheoesophageal fistula and more than half (55.3%) of them had incompetent level of total practices.
• There were a statistical significant difference between the studied nurses’ total knowledge and their ages, qualifications, years of experience and previous attending of training courses at p-value of 0.01.
• There was a statistical significant difference between the studied nurses’ total practices and their ages, qualifications, years of experience and previous attending of training courses at p-value of 0.01.
• There was positive correlation and statistically significance difference between nurses’ total knowledge and their total practice (r= 0.490. p= 0.000).
Conclusion
In the light of the current study findings, it can be concluded that, the findings answered the research questions and proved its aim. Moreover, there were more than two fifths of the studied nurses had a poor knowledge about tracheoesophageal fistula and its care. Also, there were more than half of them had incompetent practices regarding care of neonates having tracheoesophageal fistula. There were a positive correlation and a statistical significant relation between studied nurses’ total score level of knowledge and their total score level of practices.
Recommendations
In the light of the current study findings, the following recommendations were suggested:
 Periodic assessment of knowledge and practices of all nurses dealing or providing care for neonates having tracheoesophageal fistula.
 Emphasize on the importance of continuous training on appropriate nursing intervention which should be designed according to actual needs of neonates having tracheoesophageal fistula.
 Educational posters, booklets, handouts and procedures in the neonatal surgical intensive care units including pre & postoperative care regarding neonates having tracheoesophageal fistula may be more beneficial for nurses.
