Abstract

T
his study aimed to investigate the histological and immunohistochemical changes that occurred at different ages during the formation of the dental roots of the first molar of the offspring of rats born to a mother with STZ-induced diabetes.
Diabetes mellitus is characterized by metabolic disturbances and high blood sugar concentration caused by a deficiency of the hormone insulin or decreased tissue sensitivity to insulin or both.
There was no scientific evidence so far whether the blood sugar level affects the root formation of developing teeth in the generations of rats born to a diabetic mother or it affects OPN immunoexpression during root development.
Forty-two newborn Albino rats were used, they were divided into two groups: a control group and an experimental group which were further divided into three subgroups (n=7) according to date of sacrifice.
Control groups:
It is consisted of the offspring of rat mothers exposed to an intraperitoneal injection of saline. This group was divided into three subgroups, each containing seven rats, as follows:
- C.1 group sacrificed one week after birth.
- C.2 group sacrificed two weeks after birth.
- C.3 group sacrificed four weeks after birth.
Experimental groups:
They are the offspring of rat mothers exposed to a single intraperitoneal injection dose of streptozotocin of 40 mg/kg. This group was divided into three subgroups, each containing seven rats, as follows:
E.1 group sacrificed one week after birth.
E.2 group sacrificed two weeks after birth.
E.3 group sacrificed four weeks after birth.
All rats offspring were sacrificed according to the predetermined age dates and the mandible was excised and divided into right and left halves and processed to examine the first molar.
Samples from each group were prepared for examination by light microscopy as decalcified sections stained with both hematoxylin and eosin (H&E) and by immunohistochemical assay for OPN.
The main purpose of the study was to clarify the effect of induced diabetes on the formation of molar roots of offspring of rats born to mothers that were exposed to streptozotocin, through the use of:
1- Hematoxylin and eosin staining to estimate histological changes.
2- Immunohistochemical staining to study the immunohistochemical expression of OPN.
After examining the light microscopic sections of the control groups, it was found that all the different age stages underwent the normal developmental stages of the dental roots without any deviation. Examining the sections of the experimental groups, the following was found:
1- Experimental group of the first week of birth (E1):
- Presence of a thin epithelial diaphragm.
- Positive immunoreactivity of OPN was observed in the epithelial diaphragm cells in addition to pulp tissues.
2- Experimental group of the second week of birth (E2):
- Random distribution of collagen fibers of PDL was observed in addition to odontoblasts, which lack physiological adhesion from cell to cell.
- Positive immunoreactivity of OPN was observed in the cementoblastic cell layer lining the cementum tissue as well as in pulp and PDL fibers.
3- Experimental group of the fourth week of birth (E3):
- The examination revealed a destruction in the layer of odontoblasts, widening and congestion in the blood vessels in the connective tissue of the pulp.
- Hyalinized area was detected around the pulpal blood vessels.
- An apparent increase in the thickness of the cellular cementum was observed in some samples.
- Positive immunoreactivity of OPN was observed in cellular cementum, PDL fibers and pulp tissue.

CONCLUSION
• Within the limitation of this animal model study, it could be concluded that:
o Diabetes induction in rat mothers adversely affect dental root forming cells, pulp, cementum and PDL fibers of developing molar roots in the offsprings.
o D.M. was found to increase cellular cementum apposition.
o These histological changes were associated with increased OPN immuno-expression.
RECOMMENDATIONS
o Further studies should investigate large samples size at different chronological ages to extrapolate results of the present study.
o It would be of great importance to investigate in further studies cementum mineralization as well as dentin density regarding alteration that could occur in presence of induced D.M.
o Further studies investigating the effect of treated diabetic mother on odontogenesis in their offspring are required.
