Abstract

SUMMARY
This study was conducted to assess the efficacy of early oral hydration in women undergoing CS on:
1. Acceleration of the return of intestinal movement, passage of flatus and stool.
2. Evaluate the occurrence of post operative nausea and vomiting, post operative pain and abdominal distention.
3. Assess Participants satisfaction and length of hospital stay.
This randomized controlled study was conducted at Ain Shams University Maternity hospital in period from December 2020 to April 2022. It included 140 women who underwent uncomplicated Cesarean Section under regional anesthesia, and they were randomly assigned into two groups:
group I (early oral hydration) ”70 women”: They received oral fluids (within 1 hour postoperatively), beginning with water. The amounts were according to their needs, intravenous fluids were given besides and till the return of intestinal movements.
group II (control group) 70 women: these women received water after 6 hours postoperatively.
Allocation concealment done By using sealed opaque sequentially numbered envelopes, women chose one, each envelope included a random number which determine the selected group.
Randomization was performed by computer-generated random sequence (Microsoft-Seattle, WA, USA).
Women were excluded if they had Postpartum Hemorrhage, Surgical Complications Such as intestinal injury, Medical disorders such as Diabetes, Hypertension and anemia, multiple pregnancy and polyhydraminos and BMI > 30 kg/m2.
Women were subjected to written consent and they were regularly examined and asked to assess
The incidence of postoperative nausea and vomiting, abdominal distention, time of return of intestinal movements, woman satisfaction and duration of hospital stay.
The two groups were matched regarding maternal age, gestational age, parity and indication of cesarean section.
Our results showed that:
• There was statistically significant difference between two groups as regard time to first flatus (hours) when p-value was <0.05. group I (early oral hydration group) shows shorter duration to first flatus than group II (late oral hydration group).
• There was statistically significant difference between two groups as regard postoperative hospital stay (hours) when p-value was <0.05. group I (early oral hydration group) shows shorter duration of postoperative hospital stay than group II (late oral hydration group).
• There was statistically significant difference between two groups regard Overall satisfaction grade when p-value was <0.05. group I (early oral hydration group) shows more overall satisfaction than group II(late oral hydration group).
There were no significant difference regarding rates of anorexia, nausea and vomiting, time of first breast feeding. the same, There was no statistically significant difference between two groups as regard Time to first heard intestinal sounds
Although there were no significant difference among most of the study results, the early oral hydration group showed relatively better results than traditional oral hydration group as in starting breast feeding, early intestinal movements, less incidence of anorexia, nausea and vomiting.
