Abstract

Myomectomy is the major option which is the mainstay of treatment in our environment is surgery. For women failing medical management, desiring to preserve fertility and or their uterus.
The Foley catheter form of tourniquet help in reduction of blood loss during myomectomy. A foley or silicon catheter can be used and tied around the peri cervical region to stem bleeding from the uterine arteries. This has been shown to be a cheap, safe, and effective method.
The amount of blood loss is however dependent on the number, sites and volume of the myoma removed. Therefore, judicious surgical skill to minimize blood loss is also necessary especially in the black population where fibroids are more prevalent, multiple and much larger in volume.
Tranexamic acid is a safe non-hormonal medication that significantly reduces abnormal menstrual bleeding, it serves as an antifibrinolytic by reduce conversion of plasminogen to plasmin, preventing fibrin degradation and preserving the framework of fibrin’s matrix structure. Tranexamic acid also directly inhibits the activity of plasmin with weak potency and it can block the active-site of urokinase plasminogen activator.
This study intended to examine the efficacy of perioperative intravenous Tranexamic acid in further reducing blood loss at abdominal myomectomy when used as an adjunct to the haemostatic tourniquet.
The aim of this study to determine the efficacy of perioperative intravenous tranexamic acid in further reducing blood loss at abdominal myomectomy when used as an adjunct to uterine tourniquet.
This study was carried out on 50 women were assessed for study eligibility and fulfill all inclusion criteria. Study was conducted in Ain shams Maternity hospital Randomized controlled study.
The result of this study shows that there was significant useful of perioperative intravenous Tranexamic acid in further reducing blood loss at abdominal myomectomy when used as an adjunct to the haemostatic tourniquet. The objectives include the determination of intra-operative blood loss at abdominal myomectomy in women who had prophylactic intravenous Tranexamic acid plus haemostatic tourniquet and those with haemostatic tourniquet only.
The significant determination of the difference between pre-operative and post-operative packed cell volumes, comparison of intraoperative blood loss, blood transfusion as well as postoperative anaemia.
