Abstract

ABSTRACT
Background: Cerebral small vessel disease (SVD) accounts for approximately 20% of all strokes and up to half of all dementias. Although hypertension is one of the main risk factors leading to SVD, the pathogenesis of SVD is not fully understood.
Objective: To determine the age and sex-specific associations of carotid pulsatility with global burden of small vessel disease.
Patients and Methods: The patients were selected from Ain Shams University Hospitals and study included 80 patients were divided into two equal groups: First contained 40 patients group and second contained 40 normal persons (with normal MRI brain and no medical history of neurological disease) as a control group. All patients included in the study were subjected to detailed medical history and examination with detailed neurological examination, full metabolic profile, carotid duplex, TCD and MRI brain.
Results: In our study there was strong association between increasing PI of ICA and MCA with age. Age is an important predictor of arterial stiffness and cerebral SVD. As regard gender there was strong association between male sex with MCA-PI and IMT. Our study found that There is strong relation between PI of MCA and ICA with severity of cerebral SVD. But we cannot find relation between PI of CCA and severity of SVD. Also, PI of MCA and ICA were associated with age (increase more in age > 70). And MCA-PI associated with male sex.
Conclusion: Our study suggest that There is strong relation between PI of MCA and ICA with severity of cerebral SVD. But we cannot find relation between PI of CCA and severity of SVD. Also, PI of MCA and ICA associated with age (increase more in age > 70). And MCA-PI associated with male sex.
