Abstract

T
ype II odontoid fractures in elderly patients are a common type of injury, and the mortality rate is substantial in patients treated with or without surgery. However, patients undergoing surgical treatment appear to have a decreased risk of mortality at least in the first year of trauma without an increase in the risk of significant complications, with higher rate of fusion.
Randomized control trials are recommended as they are better suited to compare surgical and non-surgical treatment of odontoid fracture type II and they greatly help in decreasing bias especially selection bias which is a major cofounder in the current literature that has a distinct lack of these trials addressing this important issue.
Quality of life objective scans which facilitate accurate comparison and subsequently decision, were not clearly described in the available studies. Therefore, we recommend putting it into consideration in the future as the presence of which would greatly add to the current evidence base and would be a practical, and clinically applicable approach and would help in overcoming some of the issues.
