Abstract

This study is a retrospective statistically study including 186 females patients with ovarian cancer, admitted to female surgical ward in Oncology Centre, Mansoura University (OCMU) since 2010 till 2019, to compare the prognostic value of lymph node ratio versus the positive lymph node excised in ovarian cancer survival. All age groups included with mean age 53 +/- 9 years. All cases studied considering type of surgery either complete or incomplete surgical staging &about no. of total lymph node excised & ratio of positive to toal number excised. Recurrence has occurred to at least 48 % of cases studied within 1- 5 years with death rate among studied cases over 55% with 8% of studied cases has been lost follow up. ALL patient with total lymph node dissection have been compared acc. to effect on survival & recurrence revealed statistically insignificant difference of mean excised lymph node; positive, total and ratio, regarding survival at 1-5 years, more than 5 years and less than a year (P.>0.05 Although total lymph node dissection has better effect on recurrence with delayed recurrence incidence & improvement of disease free survival ,so it’s very crucial for all cases of early ovarian cancer to be completely staged with pelvic & para-aortic lymphadenectomy Acc. to our study, There is statistically significant higher mean total excised lymph node in cases without recurrence (14.1±15.7) than recurrent cases at 1-5 years (7.1±6.6) (P.<0.05). Considering LNR , our study has used ROC analysis about. Prognostic value of positive lymph node excised for survival revealed that Positive lymph node excised has insignificant predictive value for survival with sensitivity and specificity 11.1% and 97.7% respectively for 1-5 years survival, 96.5% and 14.1% for more than 5 years survival and 26.7% and 77.2% for less than 1 year survival (P.>0.05). Also LNR has insignificant predictive value for survival with sensitivity and specificity 21.3% and 90.5% respectively for 1-5 years survival, with cut off value > 14 lns excised & P value > 0.05.
