Abstract

Miscarriage is the most common complication of pregnancy, surgical evacuation is the standard management for termination of pregnancy, it is associated with anethetics and surgical risk such as uterine perforation, Asherman syndrome;
Medical treatment and expectant management are alternative to surgical treatment.
The most widely used regimen for induction of medical abortion involves the use of oral mifepristone200 mg followed by vaginal misoprostol 800mcg 24-48h later
However, mifepristone is expensive and is only available in many countries the advantage of misoprostol is inexpensive, stable at room temperature and available.
The aim of the work; this trial was performed to evaluate the effect of letrozole combined with misoprostol on induction of abortion compared with usage of misoprostol alone IN termination of second trimester abortion.
This study was performed on 60women diagnosed as missed abortion at Ain Shams Maternity hospital during the period from November 2018 to June 2019; the aim was to measure the effectiveness of using 3day of letrozole 10 mg tablets followed by misoprostol to induce complete abortion, and to compare the results with the complete abortion rate using misoprostol alone.
The subject population was randomly divided into groups:
1-group A (case group): 30 patients were given 10 mg letrozole orally on days1-3followed by misoprostol 200mcg vaginally every -6 hours-3 doses.
2-group B (control group): 30 patients received placebo and vaginal misoprostol with the same method the initial dose of misoprostol is (200mcg) vaginally every-6hour-3doses.
The result of the study showed complete abortion rate in 24 cases,( 80%) in women diagnosed as missed abortion in second trimester in case group compared to complete abortion in 14 case (46,7)% in control group p value( 0,007) this result showed complete abortion was significantly more frequent among case group than among control group;
This study result showed that mean induction expulsion was16,4+9,2h in case group, compared to 22,8+13,1h in control group; p value was (0,031) this result showed induction expulsion interval was significantly shorter among case group than among control group.
This study results showed failed induction in one case (3.3%) in case group and two cases (6.7%) in control group with p value (1.000) there was no significant difference between study groups regarding failed induction.
This study results showed hemoglobin before intervention case group was 11,6 gm/dl after intervention was 9,7gm/dl compared to control group hemoglobin before intervention was 11,4gm/dl after intervention was 9,6gm/dl these results showed no significant difference between the study group regarding hemoglobin.
This result showed hematocrit before intervention in case group was 38,4% after intervention was31,9% compared to control group. Hematocrit before intervention was 37,5% after intervention was 31,3% this result showed no significant difference between study group regarding hematocrit.
This study results showed number of women complaining of fever (5) and nausea (11) and vomiting (4) in case group compared to control group number of fever (3) and nausea (10) and vomiting (3) these results showed no significant difference between study groups regarding side effect.
These results indicate that letrozole show significant improvement in the efficacy of misoprostol in termination of second trimester abortion.
