Abstract

Type 1 diabetes is unique among chronic diseases of children and adolescents in terms of the complexity of treatment and level of family involvement required for successful management. Children and young people with diabetes are at a greater risk for emotional and behavioral problems, in addition to psychological aspect of the disease. Diabetic patients appear to have a greater incidence of depression, anxiety, psychological distress and eating disorders.
COVID-19 is spreading rapidly across the globe, posing a significant threat to the health of the population and creating a significant challenge for health systems, pediatric and adolescent diabetic patients are vulnerable and fragile group at increased susceptibility to infections and increased morbidity and mortality, as hyperglycemia may increase the virulence of different pathogens. They are exposed to increase levels of stress, anxiety and depression during the outbreak, together with the fear of being infected, or parents and relatives being infected.
The aim this study was to assess the psychological impact of COVID-19 pandemic (depression, anxiety symptoms, quality of life and post-traumatic stress disorder) on children and adolescents with T1DM as they are already at risk of psychological problems.
This study recruited 100 patients with T1DM from Pediatrics Hospital, Faculty of Medicine, Ain Shams University included as a case group; their age was from 7-18 years old. The study was done over period from July 2020 till November 2020. We found that 40% were having depressive symptoms according to CDI score, 67% having anxiety symptoms according to SCARED score, 47% at risk for Post-Traumatic Stress Disorder (PTSD) according to CRIES-13 score, a PedsQL (parents report) total score range from 40 – 92 with mean of 66.27 ± 12.48 and PedsQL (children report) total score range from 44-93 with mean of 68.41 ± 12.78.
Older patients were more prone to depression and post -traumatic stress disorder and lower quality of life than young age. Female patients are more prone to anxiety than male. Low quality of life was associated with increased number of siblings, frequency of DKA and high HbA1c.
There was a mutual relationship between the psychological impact of the COVID-19 pandemic and lock down and other psychological parameters, HbA1c and quality of life in pediatrics and adolescences with T1DM in Egypt.
