Abstract

In the patients with mature anterior teeth with necrotic pulp and periapical radiolucency, this study was done to evaluate the regenerative potential those teeth using blood clot(BC) and platelet rich fibrin (PRF) revascularization protocols. The efficacy of BC and PRF revascularization was evaluated by:
• Resolution of periapical radiolucency: measurement of periapical index (PAI) scores before and after the regenerative endodontic procedure(REP).
• Regaining pulp sensibility: Recoding electric pulp tester (EPT) reading before and after the REP.
Twenty patients with mature necrotic anterior teeth with periapical lesions were randomly allocated into 2 equal groups (n=10): group A; was treated by conventional revascularization protocol BC technique and group B; Revascularization was done PRF. After REP in both groups, follow up was done for one year. Periradicular healing was evaluated through standardized radiographs taken at baseline, 6, and 12 months. EPT readings were recorded at baseline and during follow-up to assess if the teeth in both groups regained sensibility during the follow-up period. Statistical analysis was done using the Mann Whitney test, Friedman test, and Wilcoxon test for non-parametric data. For parametric data, independent sample t-test, ANOVA, and paired sample t-test were used. The significance level was set at P ≤ 0.05.
The results showed that groups had no statistically significant effect. While time had a statistically significant effect and the interaction between the two variables had no statistically significant effect.
A statistically significant difference was found between pre-operative PAI scores and each of the 6th and 12th months scores respectively (p<0.001). There was no statistically significant difference among the groups after 6th where (p=0.214) and after 12th months where (p=0.081).
After one year of follow up 20% of cases in the BC group and 50%of cases in the PRF group have regained tooth sensibility.
The results also revealed a marked decrease in the readings of electrical pulp tester during the period of follow up. A statistically significant difference was found between EPT readings at baseline and the readings in the 6th and 12th months in each group (p<0.001). Also, there was a statistically significant difference between EPT readings in the 6th and 12th months in both groups (p<0.001). There was no statistically significant difference between EPT readings in the BC group and PRF group after six months(p=0.453) and one year(p=0.155) from treatment.
