Abstract Abortion is the termination of pregnancy by any means (surgical or medical) before the age of viability.The definition varies in duration of gestational age according to the countries and available facilities (Chayachinda et al., ). Estrogen is important in the maintenance of pregnancy. Aromatase inhibitors such as letrozole, suppress the peripheral conversion of androgen to estrogen, the use of letrozole combined with vaginal misoprostol was more effective than misoprostol alone in termination of pregnancy (Yeung et al., ). The present study is aimed to assess letrozole as a new synergistic agent in combination with vaginal misoprostol compared with misoprostol alone for medical abortion This clinical trial was conducted at Ain Shams University Maternity hospital in the period between December and May . Patients that fulfilling the inclusion criteria were recruited, then informed written consent was taken from every patient before starting the examination That was followed by detailed history and examination of all patients to confirm fulfilling the inclusion and exclusion criteria. Hemoglobin, blood group, RH and liver and kidney function test were done for all patients before the study. After that patients were divided into two groups; ) group A (Intervention group): women received oral letrozole mg (four tablets mg) once daily for days followed by single dose of mcg vaginal misoprostol. ) group B (controlled group): women received tablets of the placebo drug once daily for days followed by single dose of mcg vaginal misoprostol. Vital data as temperature, pulse and blood pressure were recorded every hours, side effects as abdominal pain, fever, rigors, nausea, vomiting, headache were also recorded. Pelvic examination were done every two hours to asses opening of the internal OS and to record time of complete abortion Vaginal sonography was done after hour to confirm whether the abortion is complete or not and the extent of need for curettage and evacuation. This study demonstrated that mg letrozole for three days followed by mcg of vaginal misoprostol is more effective than misoprostol alone for second trimester abortion. In the total of women were recruited the complete abortion rate of the letrozole group was significantly higher than that of the control group ( in the letrozole group compared with in the control group). No significant differences were found between the study groups regarding demographic characteristics. Induction of abortion to opening of internal os time was significantly shorter among letrozole group than among control group. Time between induction of abortion and beginning of expulsion of pregnancy products was significantly shorter among Letrozole group than among control group.Induction of abortion to complete abortion time was significantly shorter among Letrozole group than among control group. There were no significant differences between the study groups regarding side effects. Severity of vaginal bleeding was not significantly different in the letrozole group than the control group. |
