Abstract

In this study, we evaluate the role of PRF in promoting healing of temporalis fascia graft in total and subtotal tympanic membrane perforations. By comparing patients in cartilage group who underwent tympanoplasty type I using cartilage reinforced temporalis fascia graft (n=31) and patients in PRF group who underwent tympanoplasty type I using PRF clot on top of temporalis fascia graft (n=32). Both groups had good rate of graft uptake 96.9% in cartilage group and 94.2% in PRF group. No statistical difference was found between 2 groups . However, success rate of PRF group is higher than success rate of temporalis fascia graft alone as reported in the literature. The PRF decrease time needed for healing by 60% leading to earlier graft uptake and less chance for graft failure. Also, the antimicrobial action ,due to high concentration of leukocytes, may have a role in prevention of infection. Thus PRF is beneficial when added to temporalis fascia graft. Post operative hearing assessment was done at 3 months post operative. All patients had satisfactory hearing outcome with average post operative ABG 8.22±4.97 in cartilage group and 8.01±5.65 in PRF group. There was no statistical difference between the two techniques as regard average post operative ABG or frequency specific ABG. Both graft materials have satisfactory success rate and acceptable functional hearing outcome in repairing total and subtotal perforations of tympanic membrane. So, PRF can be used to promote graft uptake of temporalis fascia graft in these types of perforations.
