Abstract

Hepatocellular carcinoma (HCC) is one of the most common primary malignancies and is a leading cause of cancer-related death worldwide. Egypt has the highest prevalence of HCV worldwide and has rising rates of HCC.Liver plays a critical role in the homeostasis of lipid and lipoprotein metabolism. Lipid and lipoprotein levels could be influenced by liver diseases. To date, studies on lipid profiles in cirrhotic patients are lacking our region.The present study was designed to assess the pattern of dyslipidemia in cirrhotic patients with HCC, including the five main components of lipid profile, namely cholesterol, TG, HDL, LDL and VLDL, in addition to central obesity (measured by waist circumference), hypertension and DM. Our study also aimed to evaluate dyslipidemia as a risk factor for the development of HCC in patients with cirrhosis.This study was conducted at Al-Rajhi Liver Hospital, Assiut University Hospital, Egypt in the period between May 2017 and May 2019. It was conducted on 200 cirrhotic patients, who were divided as follows:-One hundred patients with HCC.-One hundred cirrhotic patients without HCC.All patients included in the study were subjected to- Thorough medical history and physical examination. - Laboratory investigations including the five main parameters of lipid profile (Cholesterol, TG, HDL, LDL, and VLDL), complete blood picture, liver function tests.- Abdominal ultrasonography and multi-slice abdominal CT scan commenting on liver size and hepatic focal lesions (site, size, and number).The following results were observed:- Metabolic syndrome, hypercholesterolemia, elevated serum LDL level and combined dyslipidemia were significantly higher in HCC group than non-HCC one.-Negative correlation was found between AFP and LDL levels.-Hepatomegaly, AFP, and combined dyslipidemia were associated with the presence of HCC in cirrhotic patients.-Hypercholesterolemia, hypertriglyceridemia, high LDL and combined dyslipidemia were significantly higher in HCV-related cirrhotic patients with HCC Compared to those without.- Apart from cholesterol, patients with HBV-related HCC had shown no significant difference regarding dyslipidemia in comparison to patients with HBV-related cirrhosis without HCC.- Hypertriglyceridemia was significantly higher in HCV-related cirrhotic patients with HCC Compared to those with HBV-related HCC.-No association between characters of HCC and dyslipidemia in cirrhotic patients.
