Abstract

Background: Women with polycystic ovary syndrome (PCOS) had many metabolic abnormalities that cause metabolic, cardiovascular disease risk and are associated with long term complications, so this study aimed to evaluate the effect of intermittent fasting and metformin therapy on infertile women with PCOS. Aim of this study: The aim of this study was to evaluate the effect of intermittent fasting and metformin therapy on infertile women with PCOS on their symptoms, body weight, BMI, blood pressure, hormonal pattern, glycemic, lipidemic profiles and natural conception rate. Method: A prospective randomized clinical trial was conducted at the Obstetrics and Gynecology Department of Mansoura University Hospital including 102 women with polycystic ovarian syndrome who attended the gynecological outpatient and fertility clinics and of age between 18-39 years. The duration of the study was 18 months & after taking informed consent and taking complete history, general, lacal examination, TVS ultrasongraphy and evaluation of laboratory tests after 6 months.Results: There was a significant reduction of weight and BMI with all treatment groups after 6 months, but the most reduction in weight and BMI in Metformin + intermittent fasting combined group.There was a significant reduction in diastolic blood pressure in both Intermittent fasting and combination of metformin and Intermittent fasting group.Menses irregularity was improved in Metformin and Metformin+ Intermittent fasting groups. Metformin alone and Metformin + Intermittent fasting were significaly lowered androgen level and the most reduction was in combined group.LH was significantly decreased in intermittent fasting and metformin combined group and Metformin groups .While FSH and Prolactin levels remained nearly the same in all groups.There was a significant reduction of fasting blood sugar, postprandial BS and HbA1C nearly in all groups after 6 months, but the most reduction of postprandial BS and HbA1C was in metformin combined with intermittent fasting group.S. cholesterol and TG levels was improved in metformin group and combined group. But, LDL improved only in Intermittent fasting group and Metformin group. HDL levels increased more in Intermittent fasting and Metformin combined group than other groups. There was no significant differences amoung the three groups regarding natural conception incidence. Conclusion: Combination of Metformin with intermittent fasting was associated with the highest level of improvement as it causes reduction of body weight and BMI, reduction of diastolic blood pressure, regulation of menses, reduction of androgen, lowering LH level, reduction of postprandial BS, HbA1C and improvement of HDL levels. Recommendations: It may be of additional benefit to add intermittent fasting to metformin in the treatment of some manifestations in PCOS patients. Larger studies with bigger sample sizes are needed to verify the findings of this study as regards the importance of adding intermittent fasting to metformin in the treatment PCOS patients and its consequences.KEY WORDS: polycystic ovarian syndrome, Intermittent fasting, Metformin.
