Abstract

Head injury is any sort of injury to the brain, skull, or scalp. This can range from a mild bump or bruise to a traumatic head injury. Common head injuries include concussions, skull fractures, and scalp wounds. Head injuries may be either closed or open. Traumatic head injury (THI) is a leading cause of death and disability around the globe and presents a major worldwide social, economic, and health problem. Mortality rates after head injury are highest in people with a severe THI.so the nurses’ role is essential in the prevention and early detection of this common problem with patient in intensive care unit.
Aim of the study-:
This study aims to assess the nurses’ performance regarding care of Patients with traumatic head injury in critical care unit through the following:
1- Assessment of nurses’ knowledge regarding care of Patients with traumatic head injury in intensive care unit.
2- Assessment of nurses’ practice regarding care of Patient with Traumatic head injury in intensive care unit.
Research Questions:
To fulfill the aims of this study the following, research questions were formulated:
• What are the nurses’ levels of knowledge regarding care of Patients with Traumatic head injury intensive care unit?
• What are the nurses’ levels of practices regarding care of Patients with Traumatic head injury intensive care unit?
The present study was portrayed under the four main designs as following:-
I- Technical design
II- Operational design
III- Administrative design
IV- Statistical design
1- Technical design:
The technical design includes research design, setting, subject and tools for data collection.
Research Design:
A descriptive exploratory design was conducted in study.
Setting:
The study was conducted in the intensive care unit at Ain shams University Hospital.
Subject:
Convenience sample of 40 nurses who were cared for patients with traumatic head injury in the above mentioned study setting, which was included in this study. Studied nurses were females, with different ages, different educational level and different years of experience.
Tools of data collection:
1- Self-administered questionnaire tool
This tool was divided in to two parts as follows:
Part (I):-
It was used concerned with assessment of demographic characteristics of studied nurses.
Part (II):-
This part was developed to assess nurses’ knowledge regarding traumatic head injury and the nursing management for those patients.
2- Nurses practices observational checklists:-
Researcher based on reviewing the related literature. It was used to assess Nurses’ practice related to caring for Patient with traumatic head injury in intensive care units.
It included six parts:-
I- Assessment of Glasgow coma scale.
II- Continuous blood pressure monitoring.
III- Administration of nasogastric tube feeding.
IV- Cardiopulmonary resuscitation (CPR).
V- Endotracheal tube care.
VI- Oropharynges and nasopharyngeal suction.
Results:
The results of this study have shown the following:-
• Half (50%) of the studied nurses their age between 25-40 years with a mean age of 26.59±8.45.Also half of them were having less than 5 years of experience with a mean of 9.76±5.34.Majority of the nurses(97.5%) were females. More than three quarter (82.5%) of them graduated from technical nursing education. More than half (55%) of them unmarried. Three quarter (77.5%) of them had no previous training courses.
• In relation to total knowledge about disease, three quarter (75%) of the studied nurses had unsatisfactory level about total knowledge regarding caring of traumatic head injury.
• More than three quarter (82.5%) of studied nurses had satisfactory level regarding total practice of blood pressure.
• More than half (55%) of studied nurses had unsatisfactory level regarding oropharynges and nasopharyngeal suctioning practice.
• Two thirds (67.5%) of studied nurses had satisfactory level regarding total practice about endotracheal tube care.
• More than half (52.5%) of studied nurses had satisfactory level regarding total practice about Assessment of Glasgow coma scale (GCS).
• More than three quarter (80%) of studied nurses had satisfactory level regarding total practice about Administration of nasogastric tube feeding.
• More than half (52.5%) of studied nurses had satisfactory level regarding total practice about Cardiopulmonary resuscitation (CPR).
• More than three quarter (80%) of studied nurses had satisfactory level regarding total practice about traumatic head injury.
