Abstract

The relationship between incident malignancy and the chance of developing ischemic stroke is really complicated. Cancer can lead to stroke via: hypercoagulability or direct compression of blood vessels or the various types of malignancy treatments that increase the risk of developing stroke. The study aims to clarify the relation between cancer and its treatment with ischemic stroke by discussing the different mechanisms by which a vascular insult happen causing neurological deficit. The study was conducted in Mansoura University Hospitals at Oncology and Nuclear Medicine department and Neurology Department from January 2017 till October 2018 on forty patients previously or currently suffering from manifest cancer on or not on cancer treatment who developed stroke matching forty controls with ischemic stroke. After obtaining informed consent, patients are subjected to the following:1) Detailed History taking of previous cerebrovascular insults and cancer diagnosis onset and therapies.2) Determination of major cerebrovascular risk factors 3) Physical and neurological examination.4) laboratory and radiological Investigations The results show types of cancers mostly related to the incidence of ischemic stroke in patient group are bladder and breast cancer by 22% each. The highest incidence of ischemic stroke was in patients treated with Cisplatin (42.5%).Study shows that (72.5%) of cancer patients received radiotherapy of 15 sessions or more are diagnosed with ischemic stroke. There is strong relation between incident cancer and treatment with certain chemotherapies and radiotherapy doses with high risk of ischemic stroke. The higher risk is due to hypercoagulable state of cancer or chemotherapy used, or due to vasculopathy related to treatment with radiotherapy.
