Abstract

Medical thoracoscopy (MT) is a procedure that involves access to the pleural space with an endoscope allowing direct visualization of the pleural space and intrathoracic structures while aiding in obtaining tissue or performing interventions under direct visual guidance. It increases the diagnostic yield in patients with pleural diseases when thoracocentesis and closed pleural biopsy are non-diagnostic.
In this study, we sought to evaluate the safety and Outcome of Medical Thoracoscopy in Patients Admitted to Abbasia Chest Hospital.
The present study included 123 patients with pleural effusion who presented to Abbassia Chest hospital retrospectively from October 2015 to September 2016.The study was conducted in the thoracoscopy unit in Abbassia Chest hospital.
Patients were eligible when they presented with a pleural effusion and had thoracocentesis as non conclusive diagnostic procedure.
The enrolled patients were subjected to:
1. Thoracoscopy was performed under local anesthesia.
2. All apparently abnormal areas seen were biopsied for histopathological examination.
Results showed the most common diagnosis was malignant mesothelioma, the least common diagnosis was tuberculous effusion.
Regarding to residence, there was highly significant statistical relation between malignant pleural effusion & residence in Helwan, Qaliobia & other cities rather than Cairo. While there is significant statistical relation between malignant pleural effusion & residence in Cairo, this may be due to increased industrial regions &increased pollution rate in all cities of Egypt.
Regarding to cytology, the most common cytology finding was epithelioid cells, the least common cytology finding was multicystic cells.
The most common complications were blocked drain & empyema, the least common complications are respiratory failure & atrial fibrillation.
Results showed significant relation between age & hospital stay.
Results showed significant relation between both complications &mortality and duration of drainage of ICT.
Results showed insignificant relation between diagnosis and each of sex, duration of hospital stay, duration of drainage by intercostal tube, complications & mortality.
