Abstract

W
arts are tumors or growths caused by infection with Human Papilloma virus (HPV). More than 100 HPV subtypes are known. They are common presenting disease in children and adolescents which spread by direct contact or autoinoculation.
The treatment of warts poses a therapeutic challenge for physicians. No single therapy had been proven to be effective at achieving complete remission in every patient. As a result, many different approaches to wart therapy exist. These approaches include destructive therapy, virucidal therapy, antimitotic therapy and immunotherapy (Sterling et al., 2001).
These destructive methods are variably effective; they are not always successful and may be associated with adverse effects such as painful scars, discomfort and nerve damage. Moreover, these modalities require individual treatment for each wart and do not prevent recurrence. The ideal aims of the treatment of warts should be to remove the wart without recurrence, avoid aggressive (potentially scaring) procedures, and to assist the immune system in dealing more effectively with the virus and inducing life-long immunity to HPV.
Combined furosemide and digoxin topical formula is an innovative modality of treatment in resistant multiple common warts. They interfere with k influx which is important for viral replication. Furosemide is a diuretic used in cardiac and renal diseases. Digoxin is a positive inotropic drug used in cardiac diseases.
The present study aimed to evaluate and verify the efficacy and safety of combined furosemide and digoxin as a topical therapeutic formula in multiple resistant common warts versus identical placebo formula. Our study included 20 patients, 12 males and 8 females, 18-65 years old suffering from resistant multiple common warts. The patients were equally divided into two groups; group A received furosemide and digoxin topical formula twice daily under occlusion and group B received identical placebo formula. All patients were subjected to full clinical evaluation before therapy.
In group A, the results showed that 17 warts had complete clearance and 3 had partial improvement. In group B, all the 20 warts had no change. There was no statistically significant relation found between improvement of the studied cases and their gender, age, site of the wart and duration of the disease, while there was highly statistically significant relation found between improvement of the studied cases and size of warts and duration of treatment.
In Conclusion, in this study, it was found that topically applied ionic anti viral therapy composed of furosemide and digoxin has a potential efficacy in treatment of multiple common warts regardless of patient’s age, sex, number of warts, site of warts and disease duration without any complications, side effects or recurrence.
