Abstract Kienbock’s disease, is a rare, debilitating condition that can lead to chronic pain and dysfunction. It happens when one of the eight small carpal bones in the wrist, the lunate bone, becomes damaged because there is no blood supply.It is also known as avascular necrosis of the lunate or osteocronosis of the lunate. Kienbock’s most commonly affects people aged from 20 to 40 years of age, and it occurs in men more often than in women. It rarely happens in both wrists. Symptoms include Swelling, pain, stiffness in the wrist,clicking or clunking in the wrist,weakening of grip strength,tenderness directly over the lunate bone, difficulty or pain when turning the hand upward The progression of Kienbock’s disease varies between patients, but it tends to develop slowly and subtly over a number of years.It ends by complete lunate collapse however not all patients reach that stage.many patients are diagnose in earlier stages where MRI is of great role in diagnosis. Different surgical ways of treatment are suggested for early stages of kienbocks disease. these ways include radius core decompression, joint leveling procedures and vascularized bone graft. most recent technique is the partial capitate shortening which is a way of joint leveling. |
