Abstract

The aim of Work:
This present study was done to evaluate the incidence of pericardial complications after percutaneous coronary intervention by echocardiography within 24 hours.
Methods
This study conducted in cardiology department at Ain Shams university hospital on 200 consecutive patients undergoing either emergent PCI or elective PCI starting of 10 September to 1st October 2017.
All patient had full history and examination and laboratory tests (CBC. liver function, kidney function, random blood sugar and, electrolytes and cardiac markers). All patients underwent echocardiography study within 24 hours after PCI.
Results:
In this thesis, incidence of pericardial complications after PCI was 2% of patients caused by perforation type II and III (Ellis classification) due to balloon oversizing and over inflation sealed by prolonged inflation of balloon and reversal of anticoagulant by protamine but tamponade case managed by pericardiocentesis.
Comparing other studies and this study coronary perforation and pericardial complications is a rare complication despite of our study had higher incidence the previous studies due to balloon over inflation and oversizing, using hydrophilic wires which have less tactile and more lubricant feedback.
Out of our results also, there may be significant relationship between the incidence of pericardial effusion with balloon vessel ratio.
There was no evident relationship between stent/vessel ratio and the incidence of perforation and pericardial effusion.
Also, there was no significant relationship between the incidence of pericardial complications and type of lesion (AHA/ACC classification (Type A, B and C).
There was no significant relationship between age and gender with incidence of pericardial complications after PCI.
There was also no obvious relationship between the risk factors of cardiovascular diseases (DM, HTN and smoking) with incidence of pericardial complications after PCI.
There was no relationship between the incidence of pericardial complications and the number of vessels underwent PCI.
Conclusion:
Pericardial complications after percutaneous coronary intervention is a rare complication was 2% in this study can be managed by prolonged balloon inflation in some cases and reversal of anticoagulant and may need pericardiocentesis or surgery in other cases.
