Abstract

Diabetes mellitus is a chronic disease caused by inherited and/or acquired deficiency in production of insulin by the pancreas, or by the ineffectiveness of the insulin produced. Such a deficiency results in increased concentrations of glucose in the blood, which in turn damage many of the body’s systems, in particular the blood vessels and nerves.
IDF listed Egypt among the world top 10 countries in the number of patients with diabetes. In Egypt, the prevalence of diabetes is around 15.56%. It estimated that
7.5 million individuals have diabetes and around 2.2 million have pre diabetes in Egypt. It is expected this number will jump up to 13.1 million by 2035.
In EGYPT about 42% of people with diabetes experience early-stage eye disease and 5% of people diabetes are classified as legally blind22% had peripheral neuropathy. Diabetes is also the major cause of end-stage renal disease and leg amputation in Egypt
It is estimated that only 36% of people with type 2 diabetes had HbA1c levels <7.0% ,Reasons for the difficulty in achieving target HbA1c levels may include
problems adhering to complex oral, fears about the risk of hypoglycaemia ,So health education about the disease and importance of adherence to medications is essential to achieve glycemic control.
Reorientation and motivation of health personnel towards patient education regarding diabetes to create awareness can delay the complications, improve nutrition, reduce cost, and lower the hospital admissions
It has been estimated that up to 50% of the major amputations in diabetic patients can be prevented with effective education.
General practice has the central role in type 2 diabetes management across the spectrum, from identifying those at risk right through to caring for patients at the end of life. Australian guidelines for general practioner established up-to-date guidelines, evidence-based information tailored for general practice to support general practitioners (GPs) and their teams in providing high- quality management.
Goal:
To improve the quality of life of type 2 diabetic patients.
Objectives:
1. To assess self-care activities and adherence to type 2 diabetic medications and identify barriers to adherence.
2. To develop and implement a health education program on type 2 diabetic patients to improve adherence to medications and self-care activities.
3. To find out the effect of this health education program on self-care activities, Knowledge, attitude, adherence to medications and level of HbA1C.
Subjects and Methods:
Study design: An interventional prospective one arm trial was used
Setting: The study was carried out at AIN-SHAMS Ain Shams University hospitals Diabetes Out patient’s clinic between February 2015 and May 2017.
The sample size. The starting study subjects were 140 type 2 diabetic patients plus 10% for possible DROP outs .The net number of subjects who completed the study was 125 patients.
Subjects: The patients had been selected according to the following inclusion criteria: Type 2 Diabetes Mellitus patients aged from 18 years and older, who are on oral antidiabetic drugs and duration of disease more than one year. who are attended the clinic 3 days a week.
