Abstract

There is a possibility of infertility in female cancer survivors as a side effect of the treatment modalities, as both radiotherapy and chemotherapy are gonadotoxic. So detection of ovarian reserve most sensitive measurements anti-Müllerian hormone (AMH), follicular stimulating hormone (FSH), antral follicles count (AFC) and ovarian volume (OV) before and after radiotherapy, will find out if there is a difference in ovarian reserve after radiotherapy to different body sites. These parameters could give an indication about fertility in these patients. The objective of this study was to find out if there is an effect of radiation on ovarian reserve after radiotherapy in female cancer patients. Forty female cancer patients between 18 to 40 years of age were included in our prospective cross-over study. Serum AMH and FSH were measured, also antral follicle count (AFC) and ovarian volume (OV) were measured before and after the last radiotherapy sessions by ultrasonography for all patients and measured also in control group. Psychological Resilience score was measured for patients before and after radiotherapy and measured in the control group. The control group consisted of 15 matched age healthy females to measure the same parameters. Results of the present study are highly suggestive that there is an impact on the fertility in cancer patients after cancer treatment. Serum levels of AMH decreased significantly in those patients after radiotherapy, serum levels of FSH increased significantly in all patients after radiotherapy. Also, AFC and OV decreased significantly after radiotherapy. Psychological Resilience score for patients decreased significantly after radiotherapy. Before radiation there was significant decrease in AMH and AFC in patients than in controls, significant increase in FSH in patients than controls, no significant difference in OV or Psychological resilience between patients and controls
