Abstract The aim of study is to correlate between disturbed Kappa to Lambda ratio and other prognostic parameters used in diagnosis of Multiple Myeloma as age, sex, Plasma cell count in bone marrow , B2 macroglobulin, SPEP,IF and cytogenetic and its impact on the outcome after induction. The study was conducted on 28 newly diagnosed Egyptian MM patients in Ain shams university hospitals and sheikh zaied specialized hospital. *Inclusion criteria: Patients with newly diagnosed MM. Age between 25 and 75 years old and all are with fair cardiac muscle condition All patients will be subjected to: Full history taking and clinical examination for medications and infections or systemic diseases. Thorough physical examination. Laboratory investigation including: - Complete blood count.Kidney and liver functions. - Bone marrow aspiration. - Serum protein electrophoresis. - Immunofixation. - Serum B2 microglobulin. - Serum free light chain Kappa ; Lambda ratio. - Serum calcium. - Cytogenetics (Karyotyping and/or FISH on peripheral blood or bone marrow aspirate for deletion of 17p, t(4;14) , t(11;14) hyperdiploidy ,deletion of chromosome 13. Radiological investigation including: MRI or plain X-ray to detect osteolytic lesions. The Kappa ; Lambda ratio will be correlated with the known MM diagnostic and prognostic parameters and with the course of the disease and outcome before and after induction of treatment by VCD (Velcade, cyclophosphamide, dexamethasone) or VTD(Velcade, thalidomide, dexamethasone) or VAD protocol. The aim of such study was to assess the role of Measurement of serum FLC level K;L ratio and to correlate between the level and the other diagnostic and prognostic factors used as routine investigation in diagnosis and follow up to assess the significance of this test to be enrolled as specific diagnostic mandatory test for diagnosis and follow up whoever it’s have high cost. The study show the test is highly sensitive when correlated to plasma cell level in bone marrow and have high significance when correlated to cytogenetic abnormality. (Fonseca. R, et al., 2010) Also the normal or near normal level had greater response to treatment and also had good response after Auto BMT in agreement with (Raoudha Mansouri, et al., 2016). |
