Abstract

Eighty patients were selected from patients attending Hepatology and Gastroenterology unitsatMansoura Health Insurance HospitalandBenha University Hospital in the period between JunetoJanuary2016. Seventy three of them were males and their ages ranged from 18 to 60 years.
Selected patients were divided into the following groups:group I : Included 20individual represent controls .group II: Included 30 patients with CHC .group II: Included 50patients with HCC.The aim of this work:to study the association between insulin resistance and hepatocellular carcinoma development in patients with chronic hepatitis C virus infection using HOMA – IR .
For this purpose, all patients were subjected to thorough history taking, complete clinical examination, laboratory investigations including liver pannel tests, kidney function testsand measurement of serum insulin and fasting glucose in addition to abdominal ultrasonography and CT for HCC group only.The present study found:Age was significantly higher in patients with CHC than those with HCC.There was male predomince (90%) in HCC group compared to (86.7%) in CHC group .There was a highly significant differences of the studied groups regarding Hb level , PLT, ALT,AST, total bilirubin, serum albumin, PT, INR and AFP (p- value <0.001) .
• There was a highly significant increase in the mean values of serum fasting insulin,2hour pp blood sugar and HOMA-IR in patients with HCC in comparison to patients withCHC.
• Fifty four percent of patients in HCC group were Child C , while Child A and Child B represent (16% and 30%) respectively .
