Abstract

Hereditary spherocytosis is a common inherited disorder that is characterised by a triad of anaemia, jaundice, and splenomegaly. Most patients have compensated hemolytic anemia and are asymptomatic, while others have severe hemolytic anemia requiring transfusion. Oxidative stress is not the primary etiology of most hemolytic anemias but may aggravate hemolysis.
To protect against oxidative stress, cells maintain an effective antioxidant system consisting of water- or lipid-soluble antioxidants and enzymes that remove reactive oxygen species by metabolic conversion. There is an important role of vitamin E as a lipid-soluble antioxidant in cellular membranes and lipo-proteins.
The aim of this study is to find out if vitamin E, as an antioxidant, is protective to erythrocytes in hereditary spherocytosis decreasing the oxidative stress associated with this condition. This was assessed by measuring level of vitamin E in blood in relation to biomarkers of oxidative stress and level of hemoglobin. Quality of life for those patients was additionally assessed.
A cross-sectional study including 40 children with hereditary spherocytosis recruited from the hematology clinic- Pediatric Hospital, Ain Shams University during the period from September 2017 to February 2018. They were compared to 40 apparently healthy children attending the outpatient clinic with no evidence of infection or chronic illnesses. Revision of hospital records for disease related data was done with assessment of weight and height. Laboratory investigations included blood picture, Eosin-5 maleimide test and measurement of markers of oxidative stress like GSH MDA, reduced glutathione, Superoxide dismutase, catalase, glutathione peroxidase and Vitamin E. Both the patients and caregivers were interviewed and answered the PedsQL 4.0 Generic score questionnaire.
The median age in patients group was 7years ranging from 1-18, with male to female ratio 1.05:1.There was positive consanguinity in 58.5 % of the patients and positive family history in 65.9 %. The median weight percentile was 25% ranging from 3 – 97% and median height percentile was 45% ranging from 5 – 97%.
The results of the markers of oxidative stress show highly a significant difference higher MDA, SOD, GPx, P=<0.01 in patients group and a highly significant lower GSH, catalase, vitamin E P=<0.01 in same group compared to control group. There was no significant correlation between the age of patients, age at first presentation, hemoglobin level, reticulocytic count with different markers of oxidative stress.The quality of life questionnaire showed a worse parental score in the physical scale compared to their corresponding children. There was a significant positive correlation (p=<0.05) between the hemoglobin level with the physical score in patients.
