Abstract يعتبر مرض تصلب الشرايين من أهم أسباب الوفاة عالميا. ففي مصر أكثر من 8 % مصابون بمرض تصلب الشرايين. و يعتبر مرض البول السكري من أهم عوامل الخطورة لهذا المرض. و قد اثبتت الدراسات أن الالتهاب يلعب دورا رئيسيا في عملية تصلب الشرايين. و ان نسبة الخلايا المتعادلة للخلايا الليمفاوية من المؤشرات الدالة على الالتهاب. الهدف من الدراسة هو بحث العلاقة بين الخلايا المتعادلة للخلايا اللمفاوية و مرض تصلب الشرايين التاجية باستخدام الآشعة المقطعية في مرضى البول السكري من النوع الثاني. وقد اجريت دراستنا على ستين من مرضى السكر المحولين الي مستشفيات جامعة عين شمس و مستشفى جامعة مصر للعلوم و التكنولوجيا لعمل آشعة مقطعية على الشرايين التاجية. وقد تم أخذ التاريخ المرضي و عمل الفحص السريري و الآشعة المقطعية و الفحوصات المعملية لكل المرضى المدرجين بالدراسة. و تم استبعاد المرضى الذين يعانون من التهابات حادة أو امراض مناعية الي جانب من لديهم موانع لعمل الاشعة المقطعية بالصبغة. وقد تم فحص جميع الشرايين التاجية وتقسيمها الي16مقطع تبعا للتصنيف المعدل لجمعية القلب الامريكية. وتم وصف كل مقطع اما طبيعي او به ضيق و ينقسم الضيق الي خفيف و متوسط و شديد. و قد تم التعبير عن جميع البيانات بالمتوسط الحسابي و النسبة المئوية و غيرها من الاساليب الاحصائية. و بدراسة نتائج الأحصاء وجد أن متوسط سن الفرد 57,9 ± 9,24 , و 55% كانوا من الذكور و 45,9 % يعانون من ارتفاع بضغط الدم, 45,9 % يعانون من ارتفاع نسبة الدهون بالدم, و 96,7 % لم يكن لديهم تاريخ وراثي بمرض قصور الشرايين التاجية. و وجد أن 36,1 % ليس لديهم ضيق بالشرايين التاجية , بينما 24,6 % يعانون من ضيق غير انسدادي بالشرايين التاجية و 39,3 % يعانون من ضيق انسدادي. و قد أوجدت الدراسة أنه لا توجد علاقة بين الخلايا المتعادلة للخلايا اللمفاوية و مرض تصلب الشرايين التاجية في مرضي البول السكري. Cronary artery disease (CAD) is one of the most common causes of mortality worldwide. In Egypt prevalence of CAD is more than 8%. Diabetes is one of the important risk factors of atherosclerosis. Recent evidence has demonstrated an important role of inflammation in atherosclerosis. Atherosclerosis is an active process rather than passive accumulation of lipid in the vascular lumen. Neutrophil- to- Lymphocyte ratio (NLR) is an emerging biomarker of inflammation. It is cheap and easily calculated from the neutrophil and lymphocyte count obtained from the complete blood count with differentials. The aim of our study is to investigate the relation between NLR and atherosclerotic CAD in type 2 diabetes mellitus (T2DM) patients using multisclice computed tomography (MSCT). Our study was conducted upon 60 T2DM patients presented to Ain Shams and Misr University hospitals and were referred to do MSCT for assessment of CAD. All patients were subjected to full history taking including risk factor for CAD as diabetes mellitus, hypertension, dyslipidemia, smoking and family history of premature CAD, thorough clinical examination, MSCT coronary angiography and laboratory investigations as CBC with differentials, serum creatinine and glycated hemoglobin (HbA1c). All patients signed the informed consent. Patients with acute infections, autoimmune diseases, and contraindications to do MSCT were excluded from the study. Analysis of baseline characteristics of our study population showed that mean age was 57.96 + 9.24, 55.0 % were males, 45.9 % were hypertensive, 45.9 % were dyslipidemic, 96.7 % had no positive family history of premature coronary artery disease. Of the study population, 36.1 % had no CAD by MSCT-CA, while 24.6 % exhibited non-obstructive CAD and 39.3 % showed obstructive CAD. It was found in our study that no statistical significance is present between NLR and presence of CAD. However, male gender, mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC) and red cell width (RDW) were found to be significantly associated with CAD. Also, upon studying the correlation between NLR and severity of CAD there was no statistical significance and only the MCV was associated with severity of CAD. The present study concluded that there is no significant correlation between the NLR and presence and/or severity of ischemic heart disease in T2DM patients. Other CBC indices were found to be significantly related to ischemic heart disease as MCV, MCH and RDW. Also, it was found in our study that ABOS was significantly correlated with the NLR. So, further studies with larger sample size and multiple centers are recommended. |
