Abstract

Inguinal hernia repair is one of the most commonly performed surgeries in the world.
The optimal technique for effective inguinal hernia repair is still controversial. Although open tension-free mesh techniques offer good results, the laparoscopic techniques are an effective alternative with comparable recurrence rates and complications in experienced hands, especially for bilateral and recurrent hernias.
Laparoscopic hernia repair can be done either through trans-abdominal preperitoneal approach (TAPP) or total extra-peritoneal approach (TEP). In TAPP the peritoneal cavity was entered and dissection of a peritoneal flap was created through which a mesh was introduced and placed over possible hernia sites. TEP is different in that the peritoneal cavity is not encountered and the mesh is placed over all possible hernia sites extra-peritoneal.
In this work forty cases with oblique inguinal hernia were randomly divided into two groups:
group A: 20 patients were managed by classical TAPP with narrowing of internal ring.
group B: 20 patients were managed by TAPP only.
Both groups were compared with regards to operative technique, operative time, intra & post-operative complications.
Interpretation of results revealed that laparoscopic narrowing of the internal ring added no statistical significance over classic TAPP as regards to the postoperative complications as seroma, hematoma, post- operative pain, and recurrence rate. Although follow up of the patients were short around one year, recurrence was not detected in all patients of both groups.
