Abstract

The controversy surrounding the optimal treatment for thoracolumbar injuries has changed little over the past years. Large clinical series have been published advocating both surgical and non surgical treatment for similar fracture patterns.
Thoracolumbar fractures classified to:
Pathological fracture:
A pathological fracture is a fracture that occurs through a bone that is previously weakened by disease (infection, tumor, osteoporosis)
Trumatic fracture:
Trumatic fracture occur due to sever truma to vertebral column
Methods of treatment:
 There are several methods of treatment as conservative treatment which requires the patient to stay long time in bed, this method can lead to many complication as(DVT-pneumonia-bed sores).
Surgical treatment as spinal fixation surgery which can be done by either anterior or posterior approach the study:This study was conducted upon 30 patients they were divided in to two groups:
First group: includes 15 patients suffer from dorso-lumbar fractures surgical intervention was done to them by (posterior approach).
Second group: includes 15 patients suffer from dorso-lumbar fractures surgical intervention was done to them by (anterior approach).
Follow up:
Patients were followed for a period of about two years to follow neurological state and kyphotic angels for both groups.
The result:
We have reached to conclusion from this study on both groups, that no difference between the two groups as regard to neurological state and fusion of fracture. But as regard to spinal canal diameter and kyphotic angel its seem that the anterior group more superior than the posterior group.
Anterior approach to dorso-lumbar fractures more superior than posterior approach as regard to spinal canal diameter reconstruction and correction of kyphotic angel but its need surgical skills to avoid any surgical complication.
