Abstract

Cerebral stroke is a worldwide concern, it’s classified as the second most common cause of death mainly among elderly.
Diffusion-weighted magnetic resonance imaging (MRI) can demonstrate cerebral ischemia and ischemic brain injury in the first several hours after onset of symptoms, when FLAIR and T2-weighted MRI may appear normal. It is hypothesized that these techniques could distinguish regions destined for infarction from those that will not progress to infarction.
This study is aiming to evaluate the role of DWI in detecting ischemic cerebral strokes its site, size in the patients with neurological manifestations as early as possible to help them benefit from the medical interventional management.
In this study we examined 40 patients (32 male and 8 female) with age range (24 to 80 year) suspecting stroke lesions the diffusion weighted image sequence detect 36 case positively and 4 cases were pitfalls as described.(3 false negatice and one false positive)
We observe that the false negative results of DWI were in the occipital and brain stem lesions, Vertebrobasilar stroke should therefore not be ruled out on the basis of early negative DWI, especially when symptoms persist and are suggestive of stroke diagnosis.
Transient ischemic attacks may give false positive results during examination so follow up is also recommended.
We compared our results to the recent publications in the same subject and made a comparison as regard the agreement and the disagreement points.
The DWI is a trustable technique in detecting early stroke and helps the patient from interventional management of the hyper acute stroke. Depending on directional movement of water which is determined by the brain micro-structures and imaged on the diffusion sensitive MRI.
