Abstract

A blood transfusion is the transfer of blood or blood products from one person (donor) into another person’s bloodstream (recipient). This is usually done as a life saving maneuver to replace blood cells or blood products lost through severe bleeding, during surgery when blood loss occurs or to increase the blood count in an anemic patient. Any patient receiving a transfusion of blood or blood products is at risk for a transfusion reaction. A transfusion reaction may be immediate, occurring during the transfusion or within several hours of the completion of transfusion, or delayed. Nurses have an important role in a safe blood transfusion. The aim of the study is to assess the effect of nursing guideline on nurses’ performance regarding care of patients undergoing blood transfusion. The study was conducted at all intensive care units (Geriatric, Emergency, Neurology, Chest, Cardio-thoracic Surgery and Cardiology Care Unit) in Ain-Shams University Hospital, a convenient sample of 52 nurses was recruited. Data were collected through tools of the study: 1) A questionnaire sheet for nurses’ knowledge assessment regarding care of patients undergoing blood transfusion to assess the demographic characteristics and knowledge about blood transfusion, and barriers facing the nurses during nursing care in ICUs/CCU. 2) An observation checklist for nurses’ practices regarding care of patients undergoing blood transfusion to assess practices of nurses regarding blood transfusion. 3) Nurses opinionaire sheet to assess nurses’ opinion regarding nursing guideline. The previously mentioned tools were used to evaluate the effect of guideline before, immediately after and at follow-up after 6 months of its implementation. The Results of this study revealed that there were highly statistically significant differences between nurses’ performance before and immediately after, before and at follow-up, and immediately after, and at follow-up guideline implementation. Conclusion: Nurses had unsatisfactory knowledge, and inadequate practices’ level regarding care of patients undergoing blood transfusion before nursing guideline implementation. Significant improvements were observed immediately after nursing guideline implementation in total score of nurses’ knowledge and practices, but statistically insignificant differences were detected after six months from immediately after nursing guideline implementation. Recommendations: The study recommended that, periodic in-service training programs for continuous updating of nurses’ knowledge about blood transfusion and improving their practices to prevent complications of blood transfusion.
