Abstract

Human albumin is frequently used without regard for the relationship between its pharmacological properties and the respective disease. I investigated the prescription of human albumin in Dar Al Fouad Hospital with emphasis on adherence to international guidelines.
Aim of the work:
The purpose of this trial was to determine the pattern of appropriateness of human albumin use in Dar Al Fouad Hospital and what possible economic repercussions may result from inappropriate use.
Patients and Methods:
This was a prospective observational study in which 161 patients receiving albumin (from start to end of treatment) were assessed during 12 months period (June 2011 to June 2012) at Dar Al Fouad Hospital. Data included patient demographics, ward of admission, primary reasons for prescribing albumin, and details of albumin use. The study was based on criteria for indications and end of treatment, accepted by consensus of the physicians prescribing albumin. Albumin treatment episodes were classified as appropriate or inappropriate according to these criteria.
Results:
For 247 treatment episodes in 161 patients during the twelve months period, the clinical pharmacist found 91 (36.84 %) deviations from the developed criteria. Albumin was prescribed the most at the Cardiology and Oncology departments. After analysis, 33 of them were considered as minor. Most deviations involved starting treatment too early (n = 53) or prolonged administration (n = 38).The most prevalent inappropriate indication of used albumin was for mild hypoalbuminemia. After attainment of the primary outcome, the results were preliminary presented to the Drugs and Therapeutics Committee and the Quality department. from the time of January 2014 till May 2014, the implementation of the Secondary outcome started by the development of a Drug Restriction Policy (DRP) that was based on the use of the DMAIC process.

Conclusion:
Human Albumin use in Dar Al Fouad Hospital was not completely in accordance with the international guidelines. Drug Use Evaluation is an opportunity for clinical pharmacist to help improve therapy by designing strategies to reduce the medication error and the cost or changing prescribers manners.
