Abstract

In women, trichomoniasis has a wide range of presentations, from asymptomatic to acute or chronic inflammatory disease with vaginal discharge together with malodor. It increases the risk of adverse pregnancy outcomes and common associated with human immune deficiency virus infection. The advances in diagnostic methods to reveal T. vaginalis are hopeful for, treatment and prevention of this important STD.Aim of the work: Molecular diagnosis and genotyping of T. vaginalis in patients suffering from trichomoniasis and in asymptomatic females will be done aiming to demonstrate the relation between T. vaginalis genotypes and its clinical manifestations. Method: The study was conducted on 200 patients. One hundred patients were symptomatic, and 100 patients were asymptomatic, All patients were subjected to full medical history, local speculum examination, wet mount preparation, modified Diamond culture, and Conventional PCR target ITS1 gene were used for diagnosis of T. vaginalis. For genotyping of T. vaginalis, PCR-RFLP Results: Wet mount preparation detected 6.5% (13 out of 200 cases) while by using modified Diamond culture and was 13.5 (27 cases). As regard the symptomatic positive cases. we demonstrated one actin genotype H in the studied 27 T. vaginalis isolates with no difference between symptomatic or asymptomatic groups. Conclusion: Several studies were carried out that revealed variant prevalence among women with T. vaginalis infection using different techniques. PCR method with high sensitivity and specificity may be advantageous in laboratory diagnosis of trichomoniasis even as a routine diagnostic tool. The PCR with high sensitivity may be considered as modified gold standard for STDs with limitations of microscopy and culture in diagnosis of trichomoniasis. The wide range of clinical manifestations of T. vaginalis infection in relation to the parasite virulence is still little and need further researches.
