Abstract
The majority of ophthalmic procedures are performed under local anaesthesia, cataract extraction is the most frequently performed surgery in eldery patients. There are different approaches to the delivery of local injection anesthesia for cataract surgery. The two main approaches are retrobulbar and peribulbar. The increasing use of peribulbar anaesthesia can be partly attributed to its reputation for a reduced risk of complications including retrobulbar haemorrhage , globe perforation ,optic nerve injury and respiratory arrest from brain stem anaesthesia. Critics of peribulbar anaesthesia are a lower rate of adequate block and delayed onset of anaesthesia so the use of adjuvants to local anaesthetics for ophthalmic regional anaesthesia ,focuses on the efficacy of the adjuvants improving akinesia , speed of onset and reducing block failure
