Abstract

Venous ulcers are common resulting in diminished quality of life and frequent hospitalisations, complete healing is often difficult to achieve and recurrence is extremely common MMP expression in chronic venous ulcers probably parallels the chronicity of the condition rather than propagating it. The healing of venous ulcers is dependent on the activity of MMPs. Non-healing venous ulcers are associated with greater activity MMP-2 and MMP-9 in the tissue ulcer and patient’s serum.In this thesis, thirty patients suffering from varicose vein with chronic venous leg ulcers, un-responding for traditional methods for more than 6 months recruited in a 24-month period. This group underwent measurement of (MMP-2 & MMP-9) from the ulcer tissue and human serum, at 0 day, 4 weeks and 12 weeks by ELISA kit, Doxycycline titer at 4 weeks and 12 weeks from the ulcer tissue and ulcer size at 0day, 4 weeks and 12 weeks which assessed by means of direct ulcer tracing with computerised planimetryIn this thesis, we recognized that the 3 months treatment with oral Doxycycline 100mg twice daily is capable to diminish both MMP-2 and MMP-9 expression in ulcer tissues and blood at 4 weeks and 12 weeks after treatment. This data support that Doxycycline may possibly be proficient to correct other clinical disorders other than infections, such as chronic VLUs. But, we can’t assess the effect of Doxycycline on the progress of bacterial resistance & this is considered a borderline of this research.The expression of protease leading to chronic inflammation resulting in chronic wound that is resistant to therapies. The proteases destroy growth factors and their receptors, as well as angiogenic factors. Organize of these different components by MMPs decides, to some extent, whether angiogenesis will be fortified or repressed. MMPs also contribute to the liberation of growth factors, cleave ECM proteins, and expose regions that can activate growth factor receptors. In our research, we depicted alterations in MMP-2 & MMP-9 levels throughout 12 weeks of follow-up in patients with chronic venous leg ulcers. Moreover, the Doxycycline can be used as an effective therapy for many disorders mediated to MMPs (e.g. arterial aneurysms, periodontitis and arthritis) by its anti-inflammatory and immunomodulatory properties.
