Abstract

Successful implantation depends on the uterine cavity environment and endometrial lining. Hysteroscopy is considered to be the gold standard tool in uterine cavity assessment; owing to its high sensitivity and specificity compared to HSG and TVS.
This is a randomized control study that comprised 100 infertile patients scheduled for their first ICSI cycle. They were further divided in to two groups, interventional group which underwent hysteroscopy before ICSI with normal hysteroscopic findings; and the control group.
The role of hysteroscopy was evaluated regarding, its impact on ICSI outcome in the interventional group which underwent endometrial scratching during the procedure.
The use of routine hysteroscopy is proved to be of value in improving the pregnancy outcome following ICSI even among the normal hysteroscopy group. It is found that the clinical pregnancy rate among the normal hysteroscopy group was still significantly higher compared to those who did not do hysteroscopy (66% vs. 40%). There was a significant difference in the chemical pregnancy & live birth rates also between both groups. It may be due to several factors, such as cervical dilatation, endometrial scratching & Irrigation of the cavity with saline.

In our study we have found that routine use of hysteroscopy was beneficial as:
In Interventional group, which included patients who had hysteroscopy before their ICSI cycle, biochemical pregnancy rate was 74% & clinical pregnancy rate was 66%; while in the control group, biochemical pregnancy rate was 46% & clinical pregnancy rate was 40%. Live birth rate was 36% in the control group & 62% in the interventional group. Regarding embryo transfer, it was easy in 78% of the patients of interventional group while it was easy in 32% only of the patients of the control group.
