Abstract

Summary and conclusion

The aim of this study was to measure the level of 25(OH)Vit D3 in the serum of patient with Rheumatoid Arthritis and correlate it with disease parameters specially disease activity and rheumatoid factor titre.
All patients were subjected to the following:
1-
2- Assessment of disease activity using Disease Activity Score 28(DAS28).
3- Laboratory investigations includes:
 Full blood picture&HB concentration.
 Erythrocyte sedimentation rate using Westergren method.
 Rheumatoid factor titre using Latex fixation test .
 C-reactive protein(CRP)Titre using Latex agglutination method.
 Measurment of serum 25(OH)Vit D3 by enzyme linked immunosorbant assay (ELISA).
 Measurment of serum Ca level.
5- Plain x-ray on both hands & wrists(PA view) radiographic grading of disease activity according to Larsen classification .
The result of our study were calculated, tabulated and statistically analyzed.
The findings of this study were as follows:
1-There was statistical significant decrease of serum 25(OH) vit D3 levels in RA cases as compared to controls.
3- There was highly significant difference in serum 25(OH)vit D levels in RA patients as regards DAS28 score being lower in RA patients with severe disease activity p(<0.01 ) .
4-There was statistical significant difference as regard serum 25(OH)Vit D levels and Larsen radiological severity score being lower in sever case p(<0.05).
5-There was significant negative correlation between serum 25(OH) vit D levels and clinical,laboratory parameters of disease activity( morning stiffness, disease duration, DAS, CRP, RF titre,ESR,HB%). .
6-There was significant positive correlation between serum 25(OH) vit D level and serum Ca level.
7- There were non significant correlation between serum 25(OH) vit D levels and the age, sex in RA patients(p>0.05). .
Conclusion
from our study, we suggest that 25(OH) vit D has immunomodulatory &anti-inflammatory activities in RA & is related to ,correlated with disease activity in patients with rheumatoid arthritis.

Recmommendation
Measurment of 25(OH)vit D appears to be a potentially useful laboratory measure in rheumatoid arthritis &was strongly correlated with parameters of disease activity ,So we also suggest that 25(OH)Vit D might be a useful therapeutic target in RA.
