Abstract

Otitis media is a world-wide child health problem particu­ larly among infants and young children. It is a common problem that creates a substantial amount of morbidity and large amounts of medical care leading to a considerable fraction of the nation’s heal th expenditure. The present study was initiated aiming at studying the prevalence of different types of otitis media among primary school children in Alexandria and determining some of the factors associated with its occurrence. The intelligenc~ quotient,~scholastic achievement and hearing power of the affected pupils and their relation to different t~es of otitis media were assessed. A multi-stage stratified random sample was chosen from the students of the primary schools in the different districts in Alexandria during the scholastic year 1988 - 1989. The total sample size was 3000 students. An otoscopic survey was done on the chosen sample and all cases with otitis media were referred to Sporting Student Hospital to confirm the diagnosis of suspec­ ted secretory otitis media by tympanometry and to determine its different types .The hearing power of all cases of secretory otitis media and chronic suppurative otitis media was assessed. To determine the sequelae of diseased cases, another examina­ tion by tympanometry 3 months later was carried out in Sporting Student Hospital. To study some of the epidemiological factors related to the occurrence of otitis media, a random sample (225) was ohosen from cases referred to Sporting Student Hospital. A control group (225 pupils) matched for age, sex and region was chosen from the study sample in the schools. Data from the pupils were collected through a questionnaire interview with the mother or father of each case and control. The question­ naire included: - Personal data as name, age and sex. - Data about the sooial level of the family of the student, cigarette smoking among family members and family history of ear disease. - Data about some of the factors which may be associated with the occurrence of otitis media as allergic rhinitis, season of ear discharge, hypertrophied adenoid, chronic sinusitis and tonsillitis. The scholastic achievement’of the pupils was assessed by recording the results of the ”mid - year examination.
