Abstract

CABG position could not be returned back even after higher recorded complication due to advances in new technology which enable surgeons to include higher risk cases and to identify subclinical cases.
Postoperative neurologic injury is disastrous to the surgeon before the patient and community. Aortic manipulation is accused to be a frequent leading cause of this complication via ME release.
Objectives:This work constructed to assess the effect of frequent clamping of the aorta on neurologic and myocardial outcome in addition to length of hospital stay.
Patient and methods:
Over a 2 years period, starting in July 2003, 40 from 489 patients were selected and electively underwent an isolated primary CABG procedure at EL Hussein hospital, AL Azhar University. The patients were prospectively divided into two groups, in the group I patients, the distal and proximal anastomoses were performed during a single period of ischemia by applying a total occluding clamp to the ascending aorta(the single cross-clamp technique or SCT). In the group II patients, the proximal anastmoses were performed after removal of the total occluding clamp and by applying a tangentially placed partial occluding clamp.
The patient in this work underwent full history (including education, HTN, DM, Obesity, smoking ….etc), clinical examination, and complete laboratory evaluation (CBC, ESR, CRP, liver and renal function, PT and PTT, lipid profile, and UA).
