Abstract The present study aimed at estimation of ENA-78 levels in broncholaveolar lavage fluid using fiber-optic bronchoscope (FOP), in critically ill pediatric patients with ARDS and evaluation of its prognostic and diagnostic values. The current study included 35 critically ill pediatric patients, on mechanical ventilation, fulfilling criteria of ARDS, admitted to Ain Shams PICU. Patients were furtherly classified into 10 survivors and 25 non survivors. Patients with past history of chronic pulmonary disorder and patients with cardiac lesion were excluded from the present study. All patients underwent full clinical examination, chest X-ray (AP view), calculation of acute lung injury (ALI) score, calculation of pediatric risk of mortality score III (PRISM). The details of the ventilatory management were analyzed. Laboratory investigations included; complete blood count, organ function assessment (renal and liver function tests) and serum electrolytes. A fiberoptic bronchoscpe (PENTAXLH-150P) was used to conduct bronchoscopy for all patients to study ENA-78 level in bronchoalveolar lavage fluid in ARDS patients and mechanically ventilated control group. ENA-78 was assayed in bronchoalveolar lavage by ELISA. |
