Abstract

We can conclude that indeed Technetium-99m Sestamibi underestimates myocardial viability even after its enhancement with nitrates in comparison with Thallium-201.
Regarding prognostication, again many parameters could be missed and overlooked by using Technetium-99m. Sestamibi such as increased lung uptake. A prognostic study lasting for 36 months will be initiated to confirm these results.
Recommendations;
Modification of imaging technique, quantitation of the severity of the perfusion abnormality, and additional information from gated imaging and wall motion abnormalities may, indeed, improve the ability of Sestamibi to characterize myocardial viability.
More attention should be paid to newer Technetium-based isotopes for detecting myocardial reversibility in patients with previous myocardial infarction.
Comparative studies could of value by including other imaging protocols of Thallium-201 as rest-redistribution imaging and dual isotope imaging.
